If he is a registered sex offender, his address should be on the registry. Once you know where the offender lives, contact the local Probation & Parole office and they should be able to provide you with the officer's name. If there is a problem, and the officer is not available, ask to speak to his supervisor.
However, be prepared to give facts, not assumptions or feelings. If you have info regarding the offender's behavior which could result in a violation of supervision, be ready to make a sworn statement.
